Family Navigator

We believe in providing support to families throughout various stages of life, transitions, and difficulties. Our family navigator assists families in navigating through different systems, appointments, programs, and applications related to accessing services for their children.

Our Family Navigator focuses on supporting the children and/or caregiver in the following areas:

Supporting families to target personal/family goals identified by families
Supporting family systems to create overall wellness
Teaching families how to access private and public community resources (ex FSCD, PUF,
Jordan’s Principle) and assist with helping clients build advocacy skills
Phone calls and emails to outside agencies on the client's behalf
Assisting families with filling out paper and online forms

Coaching families on how to incorporate systems at home to help with overall organization,planning, and time management
Finding appropriate resources, community programs or supports for families and their childAttending appointments with the client and caregivers
Goal setting and accountability coaching for children and families
Providing workshops and education in-services to community and private agencies

Meet Stefanie Orazietti

Registered Social Worker

Stefanie has supported students, families, and educators within the context of an inclusive educational system for 23 years. Navigating complex systems of care and support can be overwhelming so she walks side by side with families helping them along the journey of raising and caring for children. She has a passion for providing holistic support to families facing various challenges and loves to collaborate with the whole team to help reduce family stress.
